Proof requirements for a permit, license or non-driver ID

The proof you need may depend on what type of DMV document you want. 

Important: New York offers three types of documents

If you would like to apply for a Standard document that displays "NOT FOR FEDERAL PURPOSES", please review this information below. You will need 6 points of name, plus your proof of date of birth.

  • Must be issued by a Board of Health, Bureau of Vital Statistics, or US State Dept.

  • If you were born in the Bronx, Brooklyn, Manhattan, Staten Island or Queens, you can order a copy of your birth certificate from the City of New York website.

Must be in English or translated by an embassy. There are special requirements if your I-94 has a status code. 

  • F1 (student) or F2 (spouse or child of student), you must also show the original stamped DHS form I-20, with any subsequent I-20 and “notice of approval” (if appropriate). 
  • J1 (exchange visitor) or J2 (spouse or child of exchange visitor), you must also show the original stamped DHS form DS-2019 with any subsequent IAP-66 and “notice of approval” (if appropriate).
  • A1, A2, G1 or G3, you must contact the Department of State to get a driver license, learner permit, or non-driver identification card.
  • G4, you must apply for DMV services at DMV's Midtown Manhattan office.
  • I, you must bring a letter from the Foreign Press Center to DMV's Midtown Manhattan office.
Refugees and Asylees must submit paperwork to the Supervisor for approval.
