How do I get a “lifetime” driving record?

You must request that your driving record history (sometimes called a “lifetime driving record” or “lifetime abstract”) be sent to you by mail.

Only you, or someone you have given written permission to, may order this type of driving record.  You may use the General Consent for Release of Personal Information form (MV-15GC) to establish that such permission has been given.

To get your driving record history (“lifetime driving record” or “lifetime abstract”), use Request for DMV Records (form MV-15). Your request must include

  • a photocopy of your proof of identity 1
  • a personal check or money order for $10.00 payable to the "Commissioner of Motor Vehicles"

Can I get my “lifetime driving record” by phone or at a DMV office?

No.  You must request your “lifetime driving record” by mail.

 

 

  • 1. This can either be a driver license or other government issued photo ID card (you can black out your photo).
