Read the instructions (form MV-82.1) and the information about proof of identity (form ID-82). To replace your vehicle plates, registration sticker, or registration document, complete the application (form MV-82). You can use the same form to replace any registration item.
Send photocopies of your NYS photo driver license or other proof of identity that equal a value of six points or more. Do not send your original proofs of identity.
If your address has changed, write the new address on form MV-82. The DMV will change the address on that vehicle registration only. To change your address on your photo documents and other vehicle registrations, see the instructions on how to change your address or your name.
You can have your registration items sent to an address that is different from the address on your registration document. Include a separate note and clearly print the alternate address. Do not write the alternate address on form MV-82.
If your name has changed, you must visit a DMV office. You cannot change your name by mail. See the instructions on how to change your address or your name.
If you lost one vehicle plate from a set of two vehicle plates, you must surrender the plate that you have. Enclose the plate with the forms, documents and fee. The DMV will issue a new set of vehicle plates with a new plate number. Remove frames and fasteners before you surrender a vehicle plate or vehicle plates. The DMV office will not accept a vehicle plate with a frame or any fasteners attached.
If the items were lost because of a crime, ask a police agency in NYS to complete form MV-78B for you. Send the form (not a photocopy) with your application. You do not pay a fee if your vehicle plates or registration documents were lost because of a crime.

| To replace vehicle plates, or plates and documents - $28.00 For registration classes that require only one plate (for example, a motorcycle or a trailer), the fee is $15.50. You will receive new vehicle plates and new documents that show your new plate number. |
| You do not pay a fee if your vehicle plates or registration documents were lost because of a crime, and you send form MV-78B (see instructions above). |

Send all your documents and your payment to the address shown below. Your registration items will arrive within two weeks. If you do not receive the items within two weeks, contact your DMV Call Center or visit a DMV office. Your items will not arrive as quickly by international mail.
