DPR-102: Impaired Driver Program Instructor Application (PDF)
EnglishUsed to apply to become a Impaired Driver Program instructor. A separate form DPR-102 must be completed for each instructor.

DPR-103: Impaired Driver Program Application (PDF)
EnglishApplication by a sponsor agency to establish an Impaired Driver Program under Article 31 of the NYS Vehicle and Traffic Law. A resume for the IDP Director must be included with the form DPR-103. The resume must include the name, address, telephone number, DMV Client ID Number from the driver license if applicable, the educational background and the employment background of the Director.
The applicant must also submit form DPR-104 and form DPR-102 for each instructor.

DPR-104: Impaired Driver Program Service Agreement (PDF)
EnglishAll Impaired Driver Programs must agree to the terms included in form DPR-104 in order to operate an IDP. The Service Agreement defines the responsibilities of the IDP to the motorist, to the program, and to the DMV. Form DPR-104 is required to be submitted with form DPR-103 and all original application materials. The applicant must also submit a form DPR-102 for each instructor.
