If you order more than one type of inspection certificate, send a separate form and a separate payment for each type. You can put the separate orders and payments into one envelope.

Make sure that you use the correct mailing address for your order.

Make sure that the order form is complete, accurate and legible. Make your check or money order payable to "Commissioner of Motor Vehicles." Make sure that the payment is for the correct amount.

To receive your order by express mail, include an additional $10.00 in your payment.

You can call the Vehicle Safety automated response system at 1-518-474-2398 and follow the instructions. The service is available 24 hours a day, 7 days a week. You must provide your facility number to use this service.
