ATV Dealer Requirements

This form is to be used by businesses or individuals that wish to reserve a spot on the waitlist for public official emissions 1a inspection stations. This only applies to applicants applying in counties that already have more inspection stations than the Inspection Station Cap allows.

This form must accompany the Original Facility Application for a public official emissions 1a inspection station that is purchasing their license from an existing facility.

Form may only be used by “New Vehicle Dealers” and must be attached to a completed VS-1 (Original Facility Application) with all required documentation. If approved, will allow the operation of a “New Vehicle Dealership and Inspection Station” pending investigation and final approval process.

Used by a lienholder to request a duplicate MV-901 Notice of Recorded Lien.

Used by inspection stations when inspecting historical vehicles that will not be driven at night.

To be completed in addition to the Request for Business Amendment / Duplicate Certificate (form MV-253G) when a facility reports a change in location.
