For qualified auto and ATV dealers who wish to issue registration plates to customers.

Used by certified New York State motor vehicle inspectors as a resource to assist them in the proper completion of all inspections performed.

Required for all persons applying to escort drivers to a DMV road test on behalf of a driving school.

Use to certify that the Online Permit Test was supervised by a parent or guardian for applicants under 18 years old.

Use to confirm that the Online Permit Test was completed without help from others.

Use to file an appeal from a conviction and/or penalty imposed for non-criminal moving violations after a DMV Traffic Violations Bureau hearing in New York City only (see Traffic Tickets). For convictions or penalties by courts in other locations and courts, contact the court, not DMV, for appeal information.

Unrepresented motorists who do not want to appear in person for a hearing, may appear by submitting a written statement instead.

To appear by written statement, schedule a hearing and then complete and sign the Statement in Place of Personal Appearance form and mail it to the office where your hearing is scheduled to be held.

At the hearing, the judge will hear testimony, read your statement into the record and consider any evidence presented. The judge will make a finding of “guilty” or “not guilty” and mail you a decision to the address provided in the Statement.
