FS-15	Affirmation Under Section 318 (9) and 318 (11) of the New York Vehicle and Traffic Law	Must be filed with license application after a mandatory one-year revocation for an uninsured accident has been served and mandatory $750 civil penalty paid. This form required only after a revocation for uninsured accident.
