MV-902 Application for Duplicate Certificate of Title

Use this form to request a duplicate (replacement) vehicle title by mail. The duplicate title fee is $20.  When using this form to request a duplicate title by mail, you must include a photocopy of your identification.

MV-900.1 Notice of Lien - Charge Account Customer

Filed by a lienholder who has a charge account with the DMV to record a lien on a vehicle title. If you do not have a charge account, use form MV-900. A $5.00 is charged to the escrow account of the lienholder. Both lienholder and owner must sign form. If a title has already been issued, original title must be submitted with this form.
MV-35 Statement of Vehicle Owner Not Possessing a Valid Title

Use to transfer a vehicle at least eight model years old and worth less then $1250 to a vehicle dismantler, itinerant vehicle collector or certified scrap processor when the owner does not have the title.
DTF-803 Claim for Sales and Use Tax Exemption – Title/Registration Motor Vehicle, Trailer, All-Terrain Vehicle (ATV), Vessel (Boat), or Snowmobile (at NY State Department of Tax & Finance)

Use when you are claiming exemption from sales tax for any reason other than a gift (then use DTF-802) or credit for taxes paid to other states (then use DTF-804).

MV-82TON Application for Title
