DPR-103 Impaired Driver Program Application

Application by a sponsor agency to establish a Impaired Driver Program under Article 31 of the NYS Vehicle and Traffic Law.

A resume for the IDP Director must be included with the form DPR-103.  The resume must include the name, address, telephone number, DMV Client ID Number from the driver license if applicable, the educational background and the employment background of the Director.

The applicant must also submit form DPR-104 and a form DPR-102 for each instructor.
MV-2CDL Requirements for Commercial Drivers

Required for all commercial driver license (CDL) renewals.
MV-65 Vehicle Escort Driver Application

Application for certificate to operate as an escort driver for oversize vehicles. Applicants must pass a written test based on the NYS Certified Escort Manual available from DMV Offices or Call Centers.
DS-7 Request for Driver Review

Used by a person who is not a physician or a police officer to notify the DMV of a driver who has a possible medical condition that can affect the safe operation of motor vehicles.  For more information, read Report a Medical Condition.

DPR-102 Impaired Driver Program Instructor Application

Used to apply to become a Impaired Driver Program instructor.  A separate form DPR-102 must be completed for each instructor.
DPR-104 Impaired Driver Program Service Agreement

All Impaired Driver Programs must agree to the terms included in form DPR-104 in order to operate an IDP.  The Service Agreement defines the responsibilities of the IDP to the motorist, to the program, and to the DMV.  Form DPR-104 is required to be submitted with form DPR-103 and all original application materials.  The applicant must also submit a form DPR-102 for each instructor.
DTP-201 Driver Training Programs Complaint Form

Use this form to send a complaint to NYSDMV about any of the following:

  • a driver education program or instructor,
  • a driving school or instructor,
  • a point/insurance reduction program delivery agency, sponsor, or instructor, or
  • a pre-licensing course instructor.


There is no fee to file this Complaint Form. Note: To send a complaint to NY DMV about a vehicle repair, inspection or purchase, use Division of Vehicle Safety Complaint Report (VS-35).
MV-58A Certificate of Employment

Use to certify employment of junior driver. Completed by the employer of a junior driver. The junior driver carries the form when permitted to drive to and from work without a parent, guardian, driver education teacher or driving school instructor.
