MV-262 Certification of Supervised Driving

Every driver license applicant who is 16 or 17 years old at the time of the road test, and who has a junior learner permit, must present this certification to the DMV license examiner at the time of the road test. The certification must be signed by a parent or guardian.

This form is associated with the CDL A Young Adult Training Program. Applicants for a CDL Class A License who are 18, 19 or 20 years old must bring this completed certificate to the DMV office and give it to a Motor Vehicle Representative at the time of license issuance or amendment.

MV-44NC Application for Name Change ONLY on Standard Permit, Driver License or Non-Driver ID Card

MV-619 Vision Test Report

Completed by authorized vision test provider to certify a driver meets basic DMV vision standard (otherwise, use form MV-80L). May be used instead of taking vision test at a DMV office so a license can be renewed online or by mail.

MV-253G Request for Business Amendment / Duplicate Certificate

Application to change or replace a business certificate for a dealer, inspection station, repair shop or other business regulated by DMV's Division of Vehicle Safety Services. MV-58A Certificate of Employment

Use to certify employment of junior driver. Completed by the employer of a junior driver. The junior driver carries the form when permitted to drive to and from work without a parent, guardian, driver education teacher or driving school instructor. MV-44NYR Certification of Residence

Used with an application for an enhanced driver license (EDL), enhanced learner permit or enhanced non-driver photo ID card (ENDID).

MV-45 Statement of Identity by Parent/Guardian

Completed by parent of child under age 21 to provide 4 points of proof of ID for child applying for permit/license, registration/title.  The form MV-45 can also be used to provide 1 proof of residence for a child who applies for an enhanced document.  This form MUST be signed in the presence of a DMV representative.

DO NOT sign this form before bringing it to a DMV Office.  Applicant must also provide birth certificate or other acceptable proof of date of birth. See Identification Requirements.
