Used by New York residents to apply for plates or a parking permit for people with severe disabilities. Requires a medical doctor's signature and certification. Plates are issued at state and county DMV offices, but permits are issued by local governments, not DMV. Filing a false application is a crime.

Article 19-A Motor Carrier Accident and Conviction Notification Program Application. New carriers applying for 19-A status must file using this form. This form includes both a Memorandum of Understanding - Driver's Privacy Protection Act and a Request for Article 19-A Accident & Conviction Notification Account. This form replaces the MV-15AC and the MV-15MOU.
MV-253G Request for Business Amendment / Duplicate Certificate

MV-908 Salvage Vehicle Waiver

This form is to be used by insurance companies to attest that a theft recovery vehicle meets the definition pursuant to Sec. 430 of the NY Vehicle & Traffic Law for a salvage vehicle waiver. The MV-908 is a standardized form that replaces insurance company affidavits previously prepared and submitted on company letterhead for this purpose.
