If You Have the Three Digit Insurance Company Code

If you have been in a crash or accident with a vehicle registered in New York State, the quickest and easiest way to get insurance information is to search the insurance company code list on the Department of Financial Services website. The search is free and provides immediate results so you can contact the insurance company to resolve your insurance matter. The 3-digit insurance company code can be found on your accident report. Records Request Navigator

DMV can search and provide insurance information for vehicles registered in New York State only. To start your request for insurance information, visit the Records Request Navigator and include the required information: 

  • date of the accident,
  • vehicle year and make, and
  • registrant's name

You must include a copy of the Police Accident Report (form MV-104A or MV-104AN). If you do not have a police report, complete the Report of Motor Vehicle Accident (PDF) (MV-104).

DMV will perform the search and respond to you:

  • If records show that the vehicle did not have insurance coverage on the date of the accident, DMV will notify you and take appropriate action.
  • If records show that the vehicle was properly insured on the date of the accident, DMV will send you the name of the insurance company and the policy number (if available). You must then contact the insurance company to resolve your claim. 

Uninsured Vehicles

If you contacted the insurance company and the insurance company states that the vehicle was not insured, you must get a denial of insurance letter from the insurance company. Denial of insurance letters must be from the insurance company on their letterhead specifically denying coverage on the date of the accident.

DMV can revoke the driver license of the driver of the vehicle and can also take action against the registrant of the vehicle, if appropriate.

When you receive a denial of insurance letter from the insurance company, send the following items to the Insurance Services Bureau:

  1. A copy of the denial of insurance letter 
  2. A copy of the Police Accident Report (form MV-104A or MV-104AN) ORReport of Motor Vehicle Accident (PDF) (MV-104) if a police report is not available 
  3. A cover letter to request that DMV issue a revocation. (recommended)

Once these items are received, DMV will review the documentation and take appropriate action.
