Accessibility Policy

We are committed to the accessibility of the DMV website and DMV activities, programs, and services for all persons.  The specific intention of the DMV is to make sure that the website, activities, programs, and services are accessible to persons with disabilities.

We use validation tools from the World Wide Web Consortium (W3C) to review web pages for accessibility problems.

We publish our website accessibility practices and procedures on the web. (Updated September 3, 2021)

Accessible Forms

    MV-15: Request for DMV Records (PDF)

    English

    Use to request most DMV records, including driver record abstracts. See DMV Records for information on the Federal Driver's Privacy Protection Act. Use MV-198C for accident reports. You can order a driver record abstract.

    MV-45: Statement of Identity by Parent/Guardian (PDF)

    Completed by parent of child under age 21 to provide 4 points of proof of ID for child applying for permit/license, registration/title.  The form MV-45 can also be used to provide 1 proof of residence for a child who applies for an enhanced document.  This form MUST be signed in the presence of a DMV representative.

    DO NOT sign this form before bringing it to a DMV Office.  Applicant must also provide birth certificate or other acceptable proof of date of birth. See Identification Requirements.

    MV-82TON: Application for Title (PDF)

    English

    Application and instructions used when you wish to apply for a title only, and not a registration, for a 1973 or newer model vehicle. To apply for a registration and title, use form MV-82.

    MV-232: Address Change (PDF)

    English

    Use to send a change of address for your regular or enhanced driver license, non-driver photo ID card and vehicle registrations. You can also order a new document with your new address. You can change your address online immediately.

    MV-900: Notice of Lien (PDF)

    English

    Filed by lienholder to record a lien on a vehicle title. A $5.00 fee must be paid by lienholder. Both lienholder and owner must sign form. If a title has already been issued, original title must be submitted with this form.
