Title VI Non-Discrimination Policy, Procedure and Form

Policy Statement:

Title VI of the Civil Rights Act and related Nondiscrimination authorities requires that, “No person in the United States shall, on the basis of race, color, national origin, sex, age, disability, low income, and limited English proficiency (LEP), be excluded from or participation in, be denied the benefits of, or be otherwise subjected to discrimination under, any licensing program or activity receiving federal financial assistance.”  

Complaint Procedure:

Who can file a complaint?

Members of the public must file a signed, written complaint within 180 days from the date of the alleged discrimination violation.

Where do I send the complaint?

Discrimination complaints must be in writing, signed by the complainant or their representative and must include the complainant's name, address, and daytime telephone number. The completed complaint form can be mailed to:

What are the notification time frames?

Once the complaint is received, you will be notified in writing, within 10 business days of its receipt. Upon completion of an investigation, the Affirmative Action Officer will submit a determination to the Federal Motor Carrier Safety Administration. Complainants will also be notified of their right to appeal to the US Department of Transportation and/or the (FMCSA) if the complainant is not satisfied with a NYS Department of Motor Vehicles complaint determination.
