The annual custom plate renewal fee is in addition to your vehicle registration renewal fee. You will be billed for the plates every two years when you renew your registration. $25 of the annual fee supports the New York State Water Rescue Team Awareness and Research Fund.

How to Order

Requirements

  • A copy of an official document that proves you are an active member on a water rescue team
  • Current registration must be in the name of the emergency services member

 

By Mail

To order this plate by mail, use the Application for Custom Plates (PDF) (CP-30).  

Mail the completed form and your payment to Custom Plates at the address below.

 

You cannot order this plate online, by phone, or in an office.

Receiving New Plates and Registration

  • Within 10 days of when your order is processed, you will receive new documents that show your current plate number and your new plate combination.
  • Allow at least 6 weeks for the custom plates to arrive.
  • Your new registration documents will be mailed separately from your new custom plates.
  • Do not return your old plates to DMV upon receipt of the new plates.

Destroy Your Old Plates

Once you receive your new plates you will want to destroy your old plates to protect against fraudulent use. Mark each vehicle plate with a permanent marker or cut each plate into pieces. Consider separating the plates when you recycle, and putting one plate in your recycling bin each week. To avoid an inaccurate cancellation of plates, do not return your old plates to DMV.
