Getting into the Online Crash Report Sales Portal

There are two different paths to get to the Order Crash Reports Online Portal, depending on whether you already have an NY.gov ID or not.

 

Option 1: Online

Log in Using an Existing NY.gov ID

Click the "Order Crash Reports Online" button below, and, on the next page, click the "NY.gov ID" button.

You will be asked for your NY.gov ID username and password. Once you sign in, you will be asked to set up shared secrets, if you have not already. After you have set up your shared secrets, you will end up at the start of the Order Crash Reports Online Portal.

 

Sign Up for a NY.gov ID Account and Log In

Step 1: Getting to NY.gov

Click the "Order Crash Reports Online" button below, and on the next page, click the "Sign Up for NY.gov ID" button.

 

Step 2: Create a NY.gov ID

Select the account type you wish to create, and then follow the remaining prompts to create your NY.gov ID.

An email will be sent to your email address with a link that you will need to click to verify your email address.

Once your email is verified, you will be asked to create Shared Secrets, which are an additional form of security on your account. Take note of the questions and answers you set up, as you may be asked for them later.

Finally, you will be asked to pick a two-factor authentication method. Set that up, and that ends the NY.gov ID creation process.

 

Step 3: Return to the DMV Website

After creating your shared secrets and setting up two-factor authentication, you will need to return to this page. Click on the "Start Online Motorist Crash Report" button below again which will now take you to the start of the Online Motorist Report Submission Portal. You should already be logged in after creating your account and your shared secrets.

For other reports that you may need to submit/purchase in the future, you can use the steps above for logging in with an existing NY.gov ID.

Limitations of Ordering Reports Online

Reports of crashes where no one was hurt and where there was no property damage over $1,000 in value are not available for search or sale through this online transaction. You can request these reports online using the Record Request Navigator or you can mail a completed Request for Copy of Crash Report (PDF) (MV-198C) to the address listed on the form.  

Reports of crashes involving e-bikes and e-scooters are also not available for search or sale through this online transaction. You can request these reports online using the Record Request Navigator or you can mail a completed MV-198C to the address listed on the form.

 

How to Order Crash Reports

You can search, purchase, and immediately download certified crash reports online.

To search for a report, you need at least one of the following to start:

  • DMV case number (an 8-digit number)
  • Local police code
  • Date of crash
  • County of crash
  • Vehicle Identification Number (VIN)
  • Plate number of one of the involved vehicles
  • Driver license number

Some of these fields will require at least one other piece of information to continue with your search.

Once you have enough information entered to perform a search, you will be asked to certify you are legally authorized to obtain the report you are looking for. For those looking for multiple reports, you will need to complete this certification for every new search.

You can add one or more reports to your cart at one time and purchase them all in the same transaction.

You must pay both a $7.00 search fee and a $15.00 report fee for each report. You will be charged for both of these fees at the same time and receive a single confirmation email.

After you pay, you will be able to download your reports (in PDF format) inside the portal itself

 

Access Crash Reports You Already Purchased Online

You will have 7 calendar days to access reports you already purchased online. After 7 days, you will need to purchase the reports again.

You can log back in to the Online Crash Report Sales Portal to access crash reports you have already purchased within the 7-day timeframe.

Other Ways to Order a Crash Report

Order Using the Records Request Navigator

You can still order a certified copy of a crash report online if you are unable to use the method above or if you would prefer an agent of the DMV perform the search for you.

You must pay a $10 search fee and a $15 report fee for each report you order.

You must provide the

  • date of the crash
  • county where the crash occurred

and at least one of the following

  • the plate number of a vehicle involved in the crash
  • driver license number of a person who was involved in the crash
  • name and date of birth of a person who was involved in the crash

If DMV cannot find a crash based on the information you provide, we will notify you by certified letter.

Order by Mail

You may order a copy of a crash report by mail using the Request for Copy of Crash Report (PDF) (MV-198C). You must fill out as much information as possible.

Return the completed form, along with payment for the non-refundable search fee and each requested report to:
